Italy proposes replacing harm reduction with individual recovery

--- The context --- The high-level international meeting on addiction prevention and recovery was held at San Patrignano, an Italian therapeutic community presented by its hosts as a place where the meaning of the word recovery can be understood in a tangible way. The gathering brought together delegations from several countries and was co-chaired by Italy together with France, within a coalition seeking common responses to a phenomenon that combines health, family, education, work and security. The closing remarks were delivered by Undersecretary of State Alfredo Mantovano. --- The facts --- In his closing address, Mantovano thanked the community for its hospitality and praised the work of the delegations, the police forces and the department responsible for policies against drugs and other addictions. He offered special thanks to French President Emmanuel Macron and to the French partners with whom Italy co-chairs the coalition, and recalled that Prime Minister Giorgia Meloni had opened the day by describing San Patrignano as a special, unique and exceptional place. Letizia Moratti, a co-founder of the organisation, was recognised for a model that is now setting an example in Europe and around the world. The undersecretary described the Italian network with concrete figures. There are 587 public services dedicated to addiction within the national health system, employing approximately 6,000 specialists, including doctors, nurses, social workers, educators and psychologists. In 2025 alone, those services provided assistance to 130,000 people with problems linked to the use of drugs or unprescribed medications. Alongside them stand 913 therapeutic communities with a total capacity of around 14,000 places, more than 90% of which are managed by third-sector organisations, in a model combining public and private social sectors with complementary responsibilities. --- The positions --- Mantovano frankly proposed that the term harm reduction should stop being used and should be replaced by individual recovery. He explained that the first term is misleading because a person enslaved by a substance cannot be freed by continuing to be given drugs, even in gradually smaller amounts, but only by being helped to distance themselves completely from the source of their dependency. He also defended a multidisciplinary approach that takes into account each person in psychological, family, relationship, educational, social and work-related circumstances, and not only from a healthcare and pharmacological point of view. --- What remains unknown --- It is not yet known how the terminological proposal will be applied in practice at the next meetings or in the common documents of the coalition. The follow-up calendar for the Rimini Declaration, which the delegations agreed to adopt at the close of the event, has not been detailed either. Public data are still lacking on how the spread of fentanyl and synthetic opioids is evolving in each participating country and on comparable outcomes across national treatment models, beyond the Italian figures presented at the closing session in San Patrignano. --- Unanswered questions --- Will the other countries in the coalition accept replacing the term harm reduction with individual recovery in their common documents? How will the results of the alternatives to prison provided under Law no. 140 be measured once they are applied at a larger scale? What position will the European negotiation finally adopt on cannabis flowers and active ingredient limits? Will the Rimini Declaration translate into verifiable prevention commitments among young people before the next high-level meeting of this coalition takes place? --- EPM analysis --- The proposal delivered by Mantovano places the whole person at the centre rather than medical treatment alone, a reading that connects recovery with full reintegration into family life, education and work. The weight given to communities and to the third sector shows a model in which sustained accompaniment matters as much as healthcare intervention. At the same time, the change of language opens a genuine debate about how policy success is evaluated, because measuring individual recovery requires long-term outcomes and not only immediate indicators of assistance delivered by services. 📌 EPM Take: In EPM's view, the central value of the San Patrignano closing lies in having linked language, data and reintegration within a single verifiable proposal. Replacing one expression with another will only make sense if the 587 services and the 913 communities demonstrate, through public and comparable follow-up, that more people rebuild their lives and sustain that recovery over time. The test will not be a single day of speeches, but the continuity of the public and private model, the transparent application of alternatives to prison and the effective protection of minors against messages that normalise consumption.
